
Getting a My Health Record without a Medicare or DVA card
If you're not eligible for Medicare or Department of Veteran's Affairs (DVA) benefits, you can see your immunisation history and other important health information through My Health Record.
4 steps to get a My Health Record
Below is a summary of what you need to do. Click each link for more detail.
1. Apply for an individual healthcare identifier (IHI) from Services Australia, if you don't already have one.

An individual healthcare identifier (IHI) is a number that identifies you for healthcare purposes and My Health Record.
If you don’t already have an IHI, you need to complete a Request or update an Individual Healthcare Identifier form (MS003).
You will need to provide:
- one certified* document from the primary group of identification documents listed on the IHI form (such as a foreign passport)
or
- two certified* documents from the secondary group of documents.

You will receive a letter from Services Australia with your individual healthcare identifier. This may take up to 10 business days.
Once you have received this, you can register for a My Health Record.

You must print and post the registration form and certified copies of identity documents. You cannot register for your My Health Record online or by telephone.
1. Complete a registration form
If registering for yourself, print and complete the Register for a My Health Record (PDF, 412.38 KB) form.
You will need to provide 100 points of identification. Copies of your identity documents must be certified. The form provides details about what documents you can provide and who can certify them.
Complete all questions on the form.
- Question 6: Write your IHI number provided by Services Australia.
- Question 7: Write your address as recorded with Services Australia for your IHI.
- Question 9: If you want to see details of your COVID-19 vaccination, check the last box.

3. Once your application has been processed, you will receive a letter with your Identity Verification Code. You need this code to view your record.
It may take up to 10 business days for your form to be processed.

- Go to the myGov sign in page https://my.gov.au/
- Select Create an account and follow the steps to register.
4. Link your My Health Record in myGov.
This takes about 15 minutes online. Before you can do this, you must complete all 3 steps above and have your Identity Verification Code from step 3.

There are some additional steps you need to do online before you can view your My Health Record. This helps protect your sensitive health information.
You will need your:
- Identity Verification Code and
- individual healthcare identifier (IHI).
- Sign into your myGov account at https://my.gov.au/
- From the Link a service list, scroll to see My Health Record. Select the Link button for My Health Record.
- You will be taken to this web page. Select the Continue on to My Health Record button.
- Before you can access your My Health Record, you need to verify your identity using your Identity Verification Code.
Select Use my access code and then click Get Started. - Enter your Identity Verification Code, family name and date of birth, IHI number and click Next.

Click Next through the four pop-up messages.
- From the Welcome page, click on your name.
- From the menu, select Documents, and then Immunisation.
Getting an immunisation history statement
Soon you will also be able to get an immunisation history statement online from My Health Record. You can get ready by registering for a My Health Record now.
If you need an immunisation history statement now to provide evidence of a COVID-19 vaccine, you can call the Australian Immunisation Register and ask for a printed statement. It can take up to 14 days to arrive in the post.
