Please speak to your healthcare provider and ask them to upload your information. If your healthcare provider has uploaded your information but you cannot view it in your record, please contact the My Health Record Help line on 1800 723 471.

Proof of COVID-19 vaccinations

The Australian Digital Health Agency cannot access, edit, or reproduce your COVID-19 vaccination information. All appropriate advice for accessing your vaccination history can be found on the Services Australia website. This includes guidance for accessing vaccination history online and paper, and also for Medicare and non-Medicare holders.

Subpoenas or other court orders or applications

Requests for data will be individually assessed and any disclosure will be limited to the minimum necessary to satisfy the request.

Please be aware that the Agency (as the System Operator for the purposes of the My Health Records Act 2012 (Cth) (the Act)), cannot be required to disclose health information (as defined in subsections 6(1) and 6FA of the Privacy Act 1988 (Cth)) included in a healthcare recipient’s My Health Record to a court or tribunal, except in the limited circumstances set out in subsections 69(1) and (2) of the Act.

Please be aware of these limitations before seeking your order or serving your subpoena on the Agency.

The Agency can be issued with a subpoena, warrant, notice to give or produce, summons or court order. Relevant officials can request information using the methods below.
