Based on the latest ABS census, people with a long-term health condition are more likely to see three or more healthcare providers and more than 10% report that there are issues caused by a lack of communication between them. This makes the information that specialists can access and provide through digital health technology critical to continuity of care.

The My Health Record software simulator tile (above) will direct you to a login screen.
- The username is 'OnDemandTrainingUser' and should be completed for you, however you will be required to enter a password to access the session.
- The password is 'TrainMe'. Please note this password is case sensitive.
