Discharge summaries can include: the name of the hospital or facility you were discharged from pathology tests problems/diagnoses clinical overview current medications on discharge any medications you are no longer taking allergies and adverse reactions discharge diagnosis People who can access your record will be able to view this information. Find out more about who can access a record. How to find a discharge summary Get started: Log in to your My Health Record through myGov. Select the record you want to access. Select Documents from the top menu. Select Clinical Records and then select Discharge Summaries. You can search for specific documents by filtering by fields such as date, author or organisation.
