Discharge summaries

A summary of your hospital stays and follow up treatment uploaded by your healthcare provider organisations.

Keeping your hospital stays and follow-up treatment secure and in one place

A discharge summary can be shared between your healthcare providers, including your specialist and community pharmacy, to support the continued care when you are discharged.

Discharge summaries can include:

  • the name of the hospital or facility you were discharged from
  • pathology tests
  • problems/diagnoses
  • clinical overview
  • current medications on discharge
  • any medications you are no longer taking
  • allergies and adverse reactions
  • discharge diagnosis

People who can access your record will be able to view this information. Find out more about who can access a record.

How to find a discharge summary

Get started:

  1. Sign in to My Health Record:


     
  2. Select the record you want to access.
  3. Select Documents from the top menu.
  4. Select Clinical Records and then select Discharge Summaries.

You can search for specific documents by filtering by fields such as date, author or organisation.
