Add information that you'd like your healthcare providers to see

Your personal health summary is where you can add information about allergies, adverse reactions you have experienced, or medicines you are taking. This information is available for your healthcare providers, so you don’t have to remember all your information or repeat yourself every time you see one. You can update the information in your personal health summary at any time.

People who can access your record will be able to view this information. Find out more about who can access a record

How to add information to your personal health summary

Getting started: 

  1. Sign in to My Health Record:


     
  2. Select the record you want to access.
  3. Select Documents from the top menu and select Key Information I’ve Added from the drop-down.
  4. Select Personal Health Summary.

How to add an allergy or adverse reaction

  1. Select Add Allergy or Adverse Reaction.
  2. Enter the substance or agent that caused the reaction. Describe the reaction then select Save.

How to add a medication summary

  1. Select the Add Medication button. 
  2. Enter the medication name, dose information and the reason for taking the medication, then select Save.
  3. There is no limit to the number of allergies, adverse reactions or medications you can enter.
