Health check schedule and questionnaires

A list of available questionnaires about a child's health and development.

Recording information and monitoring your child's health and development with a range of tools. 

The health check schedule and questionnaires are tools that allow you to record information about your child’s health and development. Complete them before you visit your child’s healthcare providers. Healthcare providers involved in your or your child's care as well as nominated representatives and authorised representatives will be able to view health check questionnaires completed in their record.

Health check questionnaires are currently only available for residents of New South Wales and Tasmania.

How to find the health check questionnaires in your record

Get started: 

  1. Sign in to My Health Record:


     
  2. Select the record you want to access.
  3. Select Documents from the top menu.
  4. Select My Childhood Development and select Health Check Assessment Schedule.
  5. Choose your state from the drop-down list and click on Select.
  6. A list of new and completed questionnaires will be displayed. 

How to create a health check questionnaire

Get started: 

  1. On the Health Check Assessment Schedule page, select your state or territory from the drop-down menu.
  2. To complete a questionnaire, select Create.
  3. If a questionnaire has already been completed, you can see it by selecting View. You can create a new copy of the questionnaire by selecting Add New.
  4. Complete all questions in the Health Check Questionnaire before selecting Save.
  5. Select Clear at the bottom of the screen to clear the questionnaire and start again.

How to update a health check questionnaire

Get started: 

  1. Select the current state or territory from the drop-down menu to see a list of available questionnaires for the record. Completed questionnaires will show a tick.
  2. Select the questionnaire you would like to update from the list.
  3. Once the questionnaire is displayed, you can update the information and select Save.

How to create a health check questionnaire

  1. On the Health Check Assessment Schedule page, select your state or territory from the drop-down menu.
  2. To complete a questionnaire, select Create.
